For Australian chiropractic clinics
Chiropractic patient intake and consent forms
Registration and your care agreement in one link, signed on the patient’s phone before the first visit. A consent for a spinal adjustment or an X-ray goes from the patient’s record when you need it.
- Signed on the patient’s phone
- Your clinic’s own wording
- Australian data hosting
Professional access for 30 days. No card. Registration and consent stay free on every plan.

For your clinic: one link before the first visit carries the registration and your care agreement. A treatment that needs a consent of its own goes from the patient’s record. The signed PDF stays on the record and is filed to Cliniko or PracSuite when connected. Payment stays with your existing billing.
Consent before the first visit, the way chiropractic care works
At booking
Registration and your care agreement, in one link
Your clinic shares a link or QR code. The patient enters their details on their phone, checks them, and signs your care agreement before the first visit. The registration arrives on the record with an email to your clinic.
When a treatment or an X-ray needs its own consent
A treatment consent from the record
Choose the treatment on the patient record, type the area, then copy the signing link into your email or text. The patient reads your wording, marks any question, and signs.

The signed record and any question
The PDF keeps the wording exactly as signed. Your clinic can open the record, download the paperwork, and see any question that needs a response.
The intake form on their phone
Contact details, Medicare, health fund and GP, and a check before sending. These are screens from the registration your patient receives. Add up to ten questions of your own (50 on Professional) after the standard steps.

The agreement starts in five sections: about your care, the patient’s choices, their information, fees and appointments, and their agreement. Edit each into your clinic’s own words before it goes live.
The patient signs it as part of the same link, so the paperwork is done before they arrive.
A spinal adjustment consent, in your words
Treatment consents are on from the start, with spinal adjustment and X-ray as named examples. Your clinic writes the risks; the example shows the structure. The patient reads what is recommended and where, ticks the risks, can mark a question, and signs. There is no quote unless you add one to the treatment.

Questions from chiropractic clinics
Do chiropractors need a signed consent form?
The shared Code of conduct that chiropractors practise under asks practitioners to document consent appropriately, including considering the need for written consent for procedures that are of higher risk or may result in serious injury or death. Which treatments that covers is your call, with your insurer. ClientForms records the consent your clinic decides to ask for.
Can we use our own consent wording?
Yes. The care agreement starts in five sections: about your care, the patient’s choices, their information, fees and appointments, and their agreement. Edit each into your clinic’s own words before it goes live. For a treatment consent, your clinic writes what is recommended, the risks and anything else the patient agrees to. The examples show a structure, not clinical advice.
Is there a consent form for spinal adjustment or X-rays?
The chiropractic setup starts with two named examples, spinal adjustment and X-ray. They are names only: your clinic writes the risks before the first one is sent. You can add any other treatment the same way.
Can a parent or guardian sign for a child?
Yes. A parent or legal guardian can sign a treatment consent for a child. Explaining the benefits, risks and alternatives to them, including no treatment, stays with your chiropractor.
Do patients pay through ClientForms?
No. Keep taking payment the way you do now, through your practice software or your own invoicing. Nothing in the registration or consent asks the patient to pay. Chiropractic treatment consents start without a quote; you can add one to a treatment if you want the patient to sign a fee estimate.
Do patients need an account?
No. The registration and consent open from a link on the patient’s phone. Your clinic can share the link or QR code at booking, and copy a treatment consent link into your existing email or text.
Where does the signed paperwork go?
The signed PDF stays on the patient record with the wording shown at signing. It is filed to Cliniko or PracSuite when connected. Your clinic can also download it from the record.
What does it cost?
Registration and consent forms are free on every plan. New practices get Professional access for 30 days without a card. See pricing for branding and other Professional features.
