Are practice forms really free?
Yes. Practice forms are free on all plans with no submission limits. This includes consent, intake, bulk billing, and release of information forms.
Do I need a paid plan to use practice forms?
No. ClientForms practice forms are available on the free plan. Paid plans ($9 and $19/month) unlock unlimited assessment forms (PHQ-9, GAD-7, DASS-21, and 170+ more).
Can I use practice forms without Cliniko?
Yes. ClientForms practice forms work as standalone digital forms. Cliniko integration is optional — it adds automatic patient creation and PDF attachment.
How do I customise the consent form for my practice?
In your ClientForms dashboard, open the consent form editor. Seven standard sections are included — treatment consent, confidentiality, limits, fees, cancellation policy, telehealth, and consent declaration. Toggle sections on or off, edit the wording, reorder them, or add custom sections. Template variables like {session_fee} and {practice_name} pull from your practice details so the form stays current. Your customisation applies to every patient.

How it works

How do patients receive the form?
You send a ClientForms link via SMS, email, or through Cliniko before the appointment. You can also display a QR code in your waiting room. Patients open it on their phone or computer — no app download required.
What happens when a patient submits a form?
The signed form is stored in your ClientForms dashboard and you receive a notification email. If you use Cliniko, the consent PDF attaches to the patient's file and intake data creates their patient record automatically. Release of information submissions appear with the authorised details and validity period.
Can patients complete forms on their phone?
Yes. All ClientForms practice forms are mobile-first — designed to be completed comfortably on any device.
Are forms signed digitally?
Yes. Patients provide a digital signature as part of the ClientForms form. The signed PDF is stored and can be attached to their PMS record.

Onboarding workflow

How does the onboarding bundle work?
In your ClientForms dashboard, configure which forms follow intake — consent, bulk billing, and even initial assessments like the PHQ-9 or GAD-7. Patients complete the bundle on their phone at their own pace. If they stop, they receive an email link to pick up where they left off on any device. You set the bundle once and it applies to every new patient. Assessment bundling requires a Professional plan ($19/month); practice forms are always free.
Can I use a QR code in my waiting room?
Yes. Each ClientForms practice gets a permanent intake URL that works as a QR code. Patients scan it, fill in their details, and the record is created before they sit down. On a shared clinic iPad, the form resets for the next patient after submission.
What about duplicate patients?
If a ClientForms intake submission matches an existing patient by email, phone, or name and date of birth, you see a merge prompt in your dashboard with a side-by-side comparison. The patient is never blocked or warned — that is a deliberate privacy decision.

Compliance & data

How is patient data protected?
ClientForms data is encrypted in transit and at rest, hosted on Australian servers. Sensitive fields (Medicare numbers, DVA details) use a transit model — passed directly to your PMS and never stored in ClientForms.
What happens to patient data after submission?
ClientForms automatically purges practice form response details after a retention period (60 days for bulk billing, 90 days for other forms). The signed PDF remains available in your dashboard as a permanent record. Patient identity (name, date of birth, contact details) stays permanently. Transfer sensitive details to your PMS for long-term storage.
Where is data stored?
All ClientForms data is hosted on Australian infrastructure. No offshore processing or storage.
How does the Medicare transit model work?
When a patient enters their Medicare number on a ClientForms bulk billing form, it is sent directly to Cliniko via API and attached to their patient record. ClientForms does not store the Medicare number at any point.
Is this compliant with the Privacy Act?
Yes. ClientForms practice forms are designed around the Australian Privacy Principles. The transit model for sensitive data means ClientForms has minimal data exposure — PII lives in your PMS, not in a separate system.

Integration

How does the Cliniko integration work?
ClientForms connects to your Cliniko account. When a patient submits an intake form, their details create a new Cliniko patient record. Consent forms attach as PDFs to the patient's file.
What if I use a different PMS?
ClientForms practice forms work without any PMS integration. More integrations are on the roadmap — contact hello@clientforms.app to request yours.
Can someone other than the patient authorise a release of information?
Yes. ClientForms supports authorisation by a responsible person — parent, guardian, or power of attorney. The form captures who is authorising and their relationship to the patient.
Can I export form submissions?
Yes. ClientForms form submissions can be downloaded as PDFs from your dashboard.
