Consent, intake, and admin forms for psychologists

I built the onboarding side of ClientForms so a new patient can register, read and sign your consent, and complete the measures you choose, all ahead of the first session. One link. Free to start, and it works on its own or with Cliniko.

The Patient Forms hub: a registration form, your practice forms, and a pathway that combines them into one link

Free consent, patient intake, bulk billing, and release of information forms for Australian psychologists. Registration and consent sit on one link a patient opens on their own phone, ahead of the first session. Add your logo and your scored measures to the same link, and connect Cliniko so records and signed PDFs file themselves.

Everything before the first session, on one link

Set it up once. A patient works through it on their own phone, at their own pace.

Step 1 · Registration

Choose what you collect

Start from a standard field set for psychology and switch off what you do not need. First name, last name, date of birth and one contact stay on, because that is what matches a submission to the right patient.

Step 2 · AssessmentsProfessional

Add your measures to the link

On the Professional plan you can add scored questionnaires such as the PHQ-9 and GAD-7 to your registration pathway, so a new patient answers registration, consent, and your chosen measures from one link at their own pace.

Step 3 · One link

One link, new and returning patients

One registration link works for new and returning patients alike, matching each submission to the right record by contact details or name and date of birth, so you do not end up with duplicates. Send it by SMS or email, or print the QR code for the waiting room.

Forms that carry your look

Add your practice logo and choose a colour palette so your intake and consent forms carry your own look, with an optional second logo for work you run on another organisation's behalf. Your palette flows through to the onboarding link, notification emails, and the QR codes too.

Works with Cliniko.

When your practice is connected to Cliniko, a registration creates or updates the patient record and the signed consent and intake PDFs are saved straight to that patient's file. Medicare numbers transit directly and are never stored here.

ClientForms provides free practice forms for Australian psychologists, including informed consent, patient intake, bulk billing consent, and release of information. All practice forms are free on every plan with no submission limits. The consent form includes a template editor with seven standard sections; practitioners toggle sections on or off, edit the wording, and add custom sections. Template variables like session fees and practice name update automatically. The intake form creates patient records automatically from 14+ toggleable fields, shared via link, Cliniko, or a QR code in your waiting room. Bulk billing consent includes a Medicare consultation code picker. Release of information supports authorisation by the patient or a responsible person with configurable validity periods. Practitioners configure an onboarding bundle that chains practice forms and initial assessments like the PHQ-9 or GAD-7. Patients complete the bundle at their own pace; if they stop, they receive an email link to continue on any device. With Cliniko integration, patient records and consent PDFs sync automatically. Medicare numbers and government identifiers transit directly to the PMS and are never stored. Practice form data is automatically purged after the retention period; the signed PDF remains as the permanent record. All data is hosted on Australian servers. ClientForms also offers validated clinical assessment forms with instant scoring, trend tracking, and NDIS progress reports.

Common questions

Getting started

Are practice forms really free?
Yes. Practice forms are free on all plans with no submission limits. This includes consent, intake, bulk billing, and release of information forms.
Do I need a paid plan to use practice forms?
No. ClientForms practice forms are available on the free plan. Paid plans unlock the scored assessments: Starter ($9/month) keeps 3 active forms with unlimited responses, and Professional ($19/month) makes every assessment form available, PHQ-9, GAD-7 and DASS-21 included.
Can I use practice forms without Cliniko?
Yes. ClientForms practice forms work as standalone digital forms. Cliniko integration is optional; it adds automatic patient creation and PDF attachment.
How do I customise the consent form for my practice?
In your ClientForms dashboard, open the consent form editor. Seven standard sections are included: treatment consent, confidentiality, limits, fees, cancellation policy, telehealth, and consent declaration. Toggle sections on or off, edit the wording, reorder them, or add custom sections. Template variables like {session_fee} and {practice_name} pull from your practice details so the form stays current. Your customisation applies to every patient.

How it works

How do patients fill out intake forms?
You share one link by SMS, email, through Cliniko, or as a waiting-room QR code. The patient opens it on their own phone or computer, works through their details and consent at their own pace, and signs digitally. There is nothing to print, download, or scan back, and no app for the patient to install. A completed intake creates the patient record automatically, and an onboarding bundle can add initial assessments like the PHQ-9 to the same flow, so registration paperwork and scored assessments arrive together on one platform.
How do patients receive the form?
You send a ClientForms link via SMS, email, or through Cliniko ahead of the first session. You can also display a QR code in your waiting room. Patients open it on their phone or computer. No app download required.
What happens when a patient submits a form?
The signed form is stored in your ClientForms dashboard and you receive a notification email. If you use Cliniko, the consent PDF attaches to the patient's file and intake data creates their patient record automatically. Release of information submissions appear with the authorised details and validity period.
Can patients complete forms on their phone?
Yes. All ClientForms practice forms are mobile-first, designed to be completed comfortably on any device.
Are forms signed digitally?
Yes. Patients provide a digital signature as part of the ClientForms form. The signed PDF is stored and can be attached to their PMS record.

Onboarding workflow

How does the onboarding bundle work?
In your ClientForms dashboard, configure which forms follow intake: consent, bulk billing, and even initial assessments like the PHQ-9 or GAD-7. Patients complete the bundle on their phone at their own pace. If they stop, they receive an email link to pick up where they left off on any device. You set the bundle once and it applies to every new patient. Assessment bundling requires a Professional plan ($19/month); practice forms are always free.
Can I use a QR code in my waiting room?
Yes. Each ClientForms practice gets a permanent intake URL that works as a QR code. Patients scan it, fill in their details, and the record is created before they sit down. On a shared clinic iPad, the form resets for the next patient after submission.
What about duplicate patients?
If a ClientForms intake submission matches an existing patient by email, phone, or name and date of birth, you see a merge prompt in your dashboard with a side-by-side comparison. The patient is never blocked or warned; that is a deliberate privacy decision.

Compliance & data

How is patient data protected?
ClientForms data is encrypted in transit and at rest, hosted on Australian servers. Sensitive fields (Medicare numbers, DVA details) use a transit model: passed directly to your PMS and never stored in ClientForms.
What happens to patient data after submission?
ClientForms automatically purges practice form response details after a retention period (60 days for bulk billing, 90 days for other forms). The signed PDF remains available in your dashboard as a permanent record. Patient identity (name, date of birth, contact details) stays permanently. Transfer sensitive details to your PMS for long-term storage.
Where is data stored?
All ClientForms data is hosted on Australian infrastructure. No offshore processing or storage.
How does the Medicare transit model work?
When a patient enters their Medicare number on a ClientForms bulk billing form, it is sent directly to Cliniko via API and attached to their patient record. ClientForms does not store the Medicare number at any point.
Is this compliant with the Privacy Act?
Yes. ClientForms practice forms are designed around the Australian Privacy Principles. The transit model for sensitive data means ClientForms has minimal data exposure. PII lives in your PMS, not in a separate system.

Integration

How does the Cliniko integration work?
ClientForms connects to your Cliniko account. When a patient submits an intake form, their details create a new Cliniko patient record. Consent forms attach as PDFs to the patient's file.
What if I use a different PMS?
ClientForms practice forms work without any PMS integration. Contact hello@clientforms.app to request the next PMS integration.
Can someone other than the patient authorise a release of information?
Yes. ClientForms supports authorisation by a responsible person: parent, guardian, or power of attorney. The form captures who is authorising and their relationship to the patient.
