This is a free service agreement template for NDIS plan management providers in Australia. It has 17 prebuilt sections in plain language: the parties, the term and notice period, what a plan manager does, fees, invoice handling and approval, statements, communication preferences, plan information access, representatives, complaints, conflicts of interest, changeover, notices, a deliberately empty section for the provider's own terms, and a signature declaration. The wording is operational rather than legal drafting: it contains no liability, indemnity or governing-law clauses, and every section can be edited, reordered or removed. What must be in an NDIS service agreement

The NDIA Guide to Plan Management doesn't publish a template, but it is specific about what establishing an agreement involves. Their words, then the section of this template that answers them.

This arrangement should be defined and clearly captured in the service agreement prior to commencing claiming.

How long this agreement runs

This agreement starts on the day it is signed and runs while Saltbush Plan Management manages the funding in your NDIS plan.

Either of us can end it by giving 14 days notice in writing. You can change plan managers at any time, as long as the notice agreed here is given.

What must be in an NDIS service agreement for plan management?

The NDIA Guide to Plan Management expects a plan management service agreement to capture the parties, the fees, how long the agreement runs and the notice period for ending it, how invoices will be authorised before claiming begins, and terms the participant fully understands. This template gives each of those its own section: parties, term and notice, fees, invoice handling, and two plain-language choice questions for invoice approval and visibility.

Is there a free NDIS service agreement template?

Yes. The completed sample PDF on this page needs no email at all. The blank template is an editable Word document with all 17 sections; enter your email and we send it straight away.

Does an NDIS plan manager need a service agreement?

The NDIA Guide to Plan Management treats the service agreement as central to establishing plan management: it expects providers to ensure the participant or their decision maker fully understands its terms (section 3.4), to define invoice authorisation in the agreement before claiming starts (section 4.2.1), and to honour the notice periods agreed in it when a participant changes plan managers (section 3.5).

What is the plan management establishment meeting?

Section 3.3.1 of the NDIA Guide to Plan Management requires a meeting between the plan manager and the participant when plan management is established, and a maintained record of that meeting. Walking through the service agreement, agreeing the invoice-approval arrangement and capturing the signature is the natural shape of that meeting, and a signed, dated agreement is a durable record of it.

Can I use my own service agreement wording instead?

Yes. Every section of the template can be edited, reordered or removed, and the "Your terms" section is deliberately empty so your own wording, or your lawyer’s, goes in unchanged. An agreement you already have can be pasted straight over the prebuilt sections.

Is this template legal advice?

No. It is an operational skeleton covering the matters the NDIA expects the two parties to agree: fees, invoice authorisation, statements, notice and changeover. It deliberately contains no liability clause, no indemnity and no governing-law clause; wording of that kind is yours to add under "Your terms", with your own advice.

What about an NDIS participant registration form?

A service agreement usually travels with a client intake form: the participant’s NDIS number, plan start and reassessment dates, representative or nominee, support coordinator, preferred contact method and accessible-format needs. Who signs if the participant has a nominee?

Whoever signs is recorded by name and role. The signature section asks anyone signing on a participant’s behalf to give their name and the role they hold, so the record shows, for example, "plan nominee" beside the signature.
