For Australian cosmetic and plastic surgery practices

Cosmetic surgery consent and informed financial consent forms

Registration before the first consultation. Your procedure consent and written fee estimate once your surgeon is ready to send them. Each signed record keeps the signing date and time, so your rooms can see when the cooling-off period ends.

  • Signed on the patient’s phone
  • Your practice’s own wording
  • Australian data hosting

Professional access for 30 days. No card. Registration and consent stay free on every plan.

For your practice: one registration link before the first consultation, then your procedure consent and written fee estimate, sent from the patient record when your surgeon decides. The patient can mark a question before signing. The signed PDF keeps the wording, the fee estimate and the signing time, ready to download from the patient record.

From enquiry to the signed record

What your rooms and your patient see at each step. The screens use the product’s own labels with a demo practice. Your rooms send each link from your own email or text.

Fictional example: Selma Bouvier at Bayside Cosmetic Surgery

  1. Enquiry. Your rooms share the registration link or QR code when the patient enquires.
  2. Registration. The patient enters their details on their phone and checks them before sending. The registration arrives on the patient record.
  3. Consultations. Nothing else is sent while your surgeon sees the patient. The guidelines ask for two consultations before consent is signed.
  4. Consent and fee estimate. Your rooms choose the procedure, set the fees for this patient and copy the signing link into your email or text.
  5. Reading and signing. The patient reads your fee estimate and your wording, can mark a question, and signs once for both.
  6. Signed record. The signed PDF arrives on the patient record, ready to download or print.
  7. Cooling-off. Your rooms count the 7-day cooling-off from the signing date and time, then take the booking and deposit your usual way.

Follow the order the Medical Board guidelines set out

The Guidelines for registered medical practitioners who perform cosmetic surgery and procedures set when assessment, consent and payment can happen. Your practice stays responsible for meeting them. ClientForms keeps the paperwork in that order and keeps the dates.

  1. Before the first consultation

    Registration on the patient’s phone

Your rooms share a link or QR code at enquiry. The patient enters their details and checks them before sending. The registration arrives on the patient record with an email to your practice.

  2. After the consultations

Your procedure consent and written fee estimate

Choose the procedure on the patient record, set the fee lines for this patient, then copy the signing link into your email or text. Nothing is sent at registration. The patient reads your wording, marks any question, and signs once for the consent and the fee estimate.

  3. During the cooling-off period

The signing date and time on the record

The signed PDF records the date and time the patient signed, so your rooms can count the cooling-off period from it before taking a booking or deposit.

The registration on their phone

Contact details, healthcare details and a check before sending. These are screens from the registration your patient receives.

The printed record

This is the signed PDF itself, drawn by ClientForms for the demo patient. Your rooms download it from the patient record or print it for the file.

Page 1 of the signed procedure PDF for a demo rhinoplasty patient: header rows, the quote, the recommended procedure, the risks, and the signed consent
A4, set in Noto Sans. Fees, wording and patient are fictional.
  1. Who and what. The patient, the procedure, its everyday name, the surgeon, and who signed when.

  2. The fee estimate. Each fee line, the total fee and the out-of-pocket. With no rebate entered, there is no rebate line.

  3. Your wording. What your surgeon recommends and your risk wording, each marked as written by your practice.

  4. Any question marked. If the patient marks a question about a risk, the record says which one.

  5. The signature. The sentence the patient agreed to, word for word, their signature, and the date and time.

Every page carries a record reference and page numbers, so a printed copy can be matched to the record.

Questions from cosmetic surgery practices

Does ClientForms meet the cosmetic surgery guidelines for our practice?

No. Your practice remains responsible for meeting the Medical Board’s guidelines. ClientForms keeps the registration, consent and fee estimate in order, with the wording and signing time exactly as signed.

Can a patient be asked to sign at their first consultation?

Nothing is sent at registration. Your rooms send the procedure consent and fee estimate from the patient record when your surgeon decides, so the timing stays with your practice.

Can the fee estimate include the anaesthetist’s and hospital fees?

Yes. Your rooms enter each fee as its own line, and the total is added up to the cent. You can include another practitioner’s estimate as they gave it to you.

Can we use our own consent wording?

Yes. Your practice writes the procedure, risks and declarations. The examples show a structure for your wording. They do not provide clinical risk advice.

Do patients pay through ClientForms?

No. The fee estimate is something the patient reads and signs, not a payment. Keep taking deposits the way you do now, after the cooling-off period.

What does it cost?

Registration and consent forms are free on every plan. New practices get Professional access for 30 days without a card, then Professional is $19 a month.
