    Your practice shares a link or QR code. The patient enters their details on their phone and checks them before sending. The registration arrives on the record with an email to your practice.

    The quote and consent for the planned treatment

    Choose the treatment on the patient record, type the tooth or area, adjust the quote for this patient, then copy the signing link into your email or text. The patient reads the quote and your wording, marks any question, and signs once for both.

  3. Back at the practice

    The PDF keeps the wording and figures exactly as signed. Your practice can open the record, download the paperwork, and see any question that needs a response.

Questions from dental practices

Can the new patient form have our own questions?

Yes. Add up to ten questions of your own to the registration: yes or no, a date, text, or a list. They come after the standard steps, up to seven to a page.

Does the quote cover informed financial consent?

The ADA’s policy on informed financial consent asks for a written estimate of fees before treatment. Your quote lists each fee, with ADA item numbers if you add them, the total, your estimate of the health fund rebate and the out-of-pocket. The patient reads it and signs once for the quote and the consent. Rebates remain estimates.

Can we use our own consent wording for extractions, crowns or implants?

Yes. The dental setup starts with named examples (taking out a tooth, crown, implant). They are names only: your practice writes the risks before the first one is sent. Add any other treatment the same way.

Do patients pay through ClientForms?

No. Keep taking payment the way you do now, through your practice software or your own invoicing. The quote is an estimate the patient reads and signs, not a payment, and nothing in the registration asks the patient to pay.

Do patients need an account?

No. The new patient form opens from a link on the patient’s phone. Your practice can share the link or QR code at booking, and copy the signing link for a quote and consent into your existing email or text after the consult.

Where does the signed paperwork go?

The signed PDF stays on the patient record with the wording and figures shown at signing. It is filed to Cliniko or PracSuite when connected. Your practice can also download it from the record.
