Add up to ten questions of your own, such as diet history, allergies or what the client hopes to get from their visits. Each can be yes or no, a date, text, or a list. They come after the standard steps, up to seven to a page, so the form never turns into one long scroll.
The answers arrive on the client’s record with the rest of the registration.
Your consent, signed with the registration
The consent starts from a general one with three sections: confidentiality, your cancellation policy and the client’s rights. Edit each into your own words. Set your late-cancellation fee, or switch that section off, before the form goes live.
Treatment consents are off to start in the dietitian setup. If you need one for a particular treatment, one switch turns them on; your clinic writes the wording, and there is no quote unless you add one.

Can I add my own nutrition intake questions?
Yes. Add up to ten questions of your own to the registration, such as diet history, allergies or what the client hopes to get from their visits. Each is yes or no, a date, text, or a list. They come after the standard steps, up to seven to a page.
Is the consent signed before the first visit?
Yes. The consent to treatment is part of the same link as the registration, so the client signs it at booking. It starts from a general one (confidentiality, cancellations and the client’s rights) that you edit into your own words.
Can I send a consent for a particular treatment?
Yes, when you need one. Treatment consents are off to start in the dietitian setup; one switch turns them on. Your clinic writes the wording, and there is no quote unless you add one.
Do clients pay through ClientForms?
No. Keep taking payment the way you do now, through your practice software or your own invoicing. Nothing in the registration or consent asks the client to pay.
Do clients need an account?
No. The registration and consent open from a link on the client’s phone. Your clinic can share the link or QR code at booking, or copy it into your existing email or text.
Where does the signed paperwork go?
The registration and signed consent stay on the client’s record. They are filed to Cliniko or PracSuite when connected, and your clinic can download the PDF from the record.
