1The quote comes first. Each fee, the total, your estimate of the rebate and what's left to pay, with anything not included in your words. The patient ticks that they've read it.
2Risks in two groups, one tick each. This procedure's risks, then the risks of any operation, each marked as written by your practice.
3A question never stops the signature. The patient marks which risk they want to ask about. You get one email, and their record shows the question.
4Other consents, yes or no. Blood testing here. A no is recorded as a no, and they can still sign.
5One signature, at the end. Their name, the exact sentence they sign, and a typed or drawn signature. Then one screen to check before it sends.
