The consent starts from a general one with three sections: confidentiality, your cancellation policy and the patient’s rights. Edit each into your clinic’s own words. Set your late-cancellation fee, or switch that section off, before the form goes live.
The patient signs it as part of the same link, so the paperwork is done before they arrive.
A dry needling consent form, in your words
Treatment consents are on from the start, with dry needling and spinal manipulation as named examples. Your clinic writes the risks; the example shows the structure. The patient reads what is recommended and where, ticks the risks, can mark a question, and signs. There is no quote unless you add one to the treatment.

Questions from physiotherapy clinics
Do physiotherapists need a signed consent form?
The shared Code of conduct that physiotherapists practise under asks practitioners to document consent appropriately, including considering the need for written consent for procedures that are of higher risk or may result in serious injury or death. Which treatments that covers is your call, with your insurer. ClientForms records the consent your clinic decides to ask for.
Can we use our own consent wording?
Yes. The consent to treatment starts from a general one (confidentiality, cancellations and patient rights) that you edit into your clinic’s own words. For a treatment consent, your clinic writes what is recommended, the risks and anything else the patient agrees to. The examples show a structure, not clinical advice.
Is there a dry needling consent form?
The physiotherapy setup starts with two named examples, dry needling and spinal manipulation. They are names only: your clinic writes the risks before the first one is sent. You can add any other treatment the same way.
Do patients pay through ClientForms?
No. Keep taking payment the way you do now, through your practice software or your own invoicing. Nothing in the registration or consent asks the patient to pay. Physiotherapy treatment consents start without a quote; you can add one to a treatment if you want the patient to sign a fee estimate.
Do patients need an account?
No. The registration and consent open from a link on the patient’s phone. Your clinic can share the link or QR code at booking, and copy a treatment consent link into your existing email or text.
