When you administer outcome measures across multiple sessions, ClientForms tracks score trends, calculates whether change is statistically meaningful using the Reliable Change Index (RCI), and generates a professional report ready for plan reviews.

How It Works

1. Administer assessments as usual — scores are cached and tracked automatically

2. Open a client's NDIS tab — see score trends charted with severity band overlays

3. Click "Generate Report" — choose DOCX or PDF format

4. Review and send — the report includes participant details, functional impact summary, score comparison tables, and clinical recommendations
