Start from a standard field set and switch off what you do not need. First name, last name, date of birth and one contact stay on, because that is what matches a submission to the right client. Turn on NDIS or funding details when you need them.

All practice forms are free on every plan with no submission limits. The consent form includes a template editor; practitioners toggle sections on or off, edit the wording, and add custom sections. Template variables like session fees and practice name update automatically. The client intake form creates records automatically from a toggleable field set, including optional NDIS and funding fields, shared via link, Cliniko, or a QR code in your waiting room. Bulk billing consent includes a Medicare consultation code picker. Getting started

Are practice forms really free?
Yes. Practice forms are free on all plans with no submission limits. This includes consent, client intake, bulk billing, and release of information forms.
Do I need a paid plan to use practice forms?
No. ClientForms practice forms are available on the free plan. Paid plans unlock scored measures in the onboarding link: Starter ($9/month) keeps 3 active forms with unlimited responses, and Professional ($19/month) makes every form available, including self-report outcome measures such as the FES-I.
Can I use practice forms without Cliniko?
Yes. ClientForms practice forms work as standalone digital forms. Cliniko integration is optional; it adds automatic client creation and PDF attachment.
How do I customise the consent form for my practice?
In your ClientForms dashboard, open the consent form editor. Toggle standard sections on or off, edit the wording, reorder them, or add your own. Template variables like {session_fee} and {practice_name} pull from your practice details so the form stays current. Your customisation applies to every client.
Can I collect NDIS and funding details on intake?
Yes. The client intake form has a standard field set you trim to fit, and you can switch on NDIS number, plan management type, and funding details when they are relevant. Fields you do not use stay switched off, so clients only see what applies to them.

How it works

How do clients fill out intake forms?
You share one link by SMS, email, through Cliniko, or as a waiting-room QR code. The client opens it on their own phone or computer, works through their details and consent at their own pace, and signs digitally. There is nothing to print, download, or scan back, and no app for the client to install. A completed intake creates the client record automatically, and an onboarding bundle can add your chosen forms to the same flow.
How do clients receive the form?
You send a ClientForms link via SMS, email, or through Cliniko ahead of the first appointment. You can also display a QR code in your waiting room. Clients open it on their phone or computer. No app download required.
What happens when a client submits a form?
The signed form is stored in your ClientForms dashboard and you receive a notification email. If you use Cliniko, the consent PDF attaches to the client's file and intake data creates their record automatically. Release of information submissions appear with the authorised details and validity period.
Can clients complete forms on their phone?
Yes. All ClientForms practice forms are mobile-first, designed to be completed comfortably on any device.
Are forms signed digitally?
Yes. Clients provide a digital signature as part of the ClientForms form. The signed PDF is stored and can be attached to their PMS record.

Onboarding workflow

How does the onboarding bundle work?
In your ClientForms dashboard, configure which forms follow intake: consent, bulk billing, and your self-report outcome measures such as the FES-I. Clients complete the bundle on their phone at their own pace. If they stop, they receive an email link to pick up where they left off on any device. You set the bundle once and it applies to every new client. Bundling scored measures requires a Professional plan ($19/month); practice forms are always free.
Can I use a QR code in my waiting room?
Yes. Each ClientForms practice gets a permanent intake URL that works as a QR code. Clients scan it, fill in their details, and the record is created before they sit down. On a shared clinic iPad, the form resets for the next client after submission.
What about duplicate clients?
If a ClientForms intake submission matches an existing client by email, phone, or name and date of birth, you see a merge prompt in your dashboard with a side-by-side comparison. The client is never blocked or warned; that is a deliberate privacy decision.

Compliance & data

How is client data protected?
ClientForms data is encrypted in transit and at rest, hosted on Australian servers. Sensitive fields such as Medicare numbers use a transit model: passed directly to your PMS and never stored in ClientForms.
What happens to client data after submission?
ClientForms automatically purges practice form response details after a retention period (60 days for bulk billing, 90 days for other forms). The signed PDF remains available in your dashboard as a permanent record. Client identity (name, date of birth, contact details) stays permanently. Transfer sensitive details to your PMS for long-term storage.
Where is data stored?
All ClientForms data is hosted on Australian infrastructure. No offshore processing or storage.
Is this compliant with the Privacy Act?
Yes. ClientForms practice forms are designed around the Australian Privacy Principles. The transit model for sensitive data means ClientForms has minimal data exposure. PII lives in your PMS, not in a separate system.

Integration

How does the Cliniko integration work?
ClientForms connects to your Cliniko account. When a client submits an intake form, their details create a new Cliniko client record. Consent forms attach as PDFs to the client's file. Cliniko integration is available in Australia and New Zealand.
What if I use a different PMS?
ClientForms practice forms work without any PMS integration. Contact hello@clientforms.app to request the next PMS integration.
Can someone other than the client authorise a release of information?
Yes. ClientForms supports authorisation by a responsible person: parent, guardian, or power of attorney. The form captures who is authorising and their relationship to the client, and you can release to a third party such as a GP, plan manager or school.
