Open Consent & Agreements

From the Patient Forms hub, click Consent & Agreements. This shows all your consent templates.

Click + New Agreement

Start a new template. Give it a name that makes sense for your practice — "Psychology Consent", "Telehealth Consent", or something specific like "LCA Pastoral Ministry Consent".

Add sections

Each section covers a different part of your consent (confidentiality, fees, cancellation policy, mandatory reporting). Add as many as you need. Drag to reorder.

Preview the patient view

Switch to preview mode to see exactly what the patient sees — including the signature capture at the bottom.

Click Save. Your consent template is ready to use.

The consent template now appears as an option in the pathway editor. Attach it to any intake pathway so patients sign it during registration.

Frequently asked questions

Can I have multiple consent templates?
Yes, on the Professional plan. Create different templates for different purposes — one for standard psychology consent, another for telehealth, another for specific assessment types. Each intake pathway can use a different template.
How do patients sign the consent form?
Patients draw their signature on screen (touch or mouse). The signed consent is stored with a timestamp and can be viewed from the patient's record on your dashboard.
