Consent forms are available for Australian psychology and occupational therapy practices. Multiple templates require the Professional plan.

Open Consent & Agreements

From the Patient Forms hub, click Consent & Agreements. This shows all your consent templates.

Click + New Agreement

Start a new template. Give it a name that makes sense for your practice — "Psychology Consent", "Telehealth Consent", or something specific like "Child Assessment Consent".

Add sections

Each section covers a different part of your consent (confidentiality, fees, cancellation policy, mandatory reporting). Add as many as you need. Use the arrow buttons to reorder. Insert variable tokens like {{practice_name}} that auto-fill from your Practice Profile.

Template available in pathways

The consent template now appears as an option in the pathway editor. Attach it to any intake pathway so patients sign it during registration.

Frequently asked questions

Can I have multiple consent templates?
Yes, on the Professional plan. Create different templates for different purposes — one for standard psychology consent, another for telehealth, another for specific assessment types. Each intake pathway can use a different template.
How do patients sign the consent form?
Patients draw their signature on screen (touch or mouse). Some templates also include typed signature confirmations ('I, [Name], acknowledge...') alongside the drawn signature. Signed consents are stored with a timestamp and can be viewed from the patient's record.
Can I add my practice variables to the consent text?
Yes. Insert variable tokens like {{session_fee}} or {{cancellation_notice}} into your section text. These are replaced with your Practice Profile values when patients view the form.
