Creating custom forms requires the Professional plan.

Open Patient Forms

From your dashboard sidebar, click Patient Forms. Navigate to Step 2 — Forms.

Click + New Form

At the bottom of your forms list, click + New Form. Choose "Agreement" for a pre-loaded template with 4 sections and a signature, or "Start Blank" for an empty form.

Name your form

Give it a clear name — "Telehealth Consent", "NDIS Service Agreement", "Release of Information". This is what patients and your dashboard will show.

Add sections with rich text

Each section has a title and a rich text body. Write your content, insert variable tokens like {{practice_name}} or {{session_fee}} that auto-fill from Practice Profile. Use the arrow buttons to reorder sections.

Click Save. Your form is stored as a draft — visible to you but not to patients. Come back anytime to keep editing.

Publish when ready

Toggle to Published. Your form is now available for use in intake pathways or as a standalone link. Copy the link or generate a QR code from the share card.

Frequently asked questions

What is the difference between draft and published?
A draft form is saved but not visible to patients. Publishing makes it available in pathways and as a standalone link. You can unpublish at any time.
Can I use variables like practice name or patient name?
Yes. Insert variable tokens (e.g. {{practice_name}}, {{session_fee}}) into your section text. These are replaced with real values from your Practice Profile when the patient views the form.
What form types are available?
You can create consent agreements, billing forms, authorisation forms, practice policies, and referral forms. Each type uses the same editor but is categorised separately on your dashboard.
