Set up a patient intake pathway

Combine intake, consent, admin forms, and assessments into a single link patients complete before their first appointment.

Intake pathways are currently available for Australian psychology practices. Custom pathways require the Professional plan. OT and US region support coming soon.

Open Intake Pathways

From the Patient Forms hub, click Intake Pathways. This shows your existing pathways or lets you create a new one.

Create a new pathway

Click + New Pathway and give it a name — "New Patient Onboarding" or "Telehealth Intake". You can have multiple pathways for different purposes.

Choose registration type

Pick how patients register: Full Intake Form (collects detailed information) or Basic Details (name, email, date of birth only). Most practices use Full Intake Form.

Select a consent template

Choose which consent template patients sign. If you haven't created one yet, you can skip this and add it later.

Add admin forms

Optionally add billing and admin forms — bulk billing consent, release of information, funding consent. Patients complete these after consent.

Add assessments

Browse the assessment catalogue or select a battery. These are the clinical assessments patients complete at the end of onboarding. Optional — not every pathway needs assessments.

Save and activate

Click Save. Toggle the pathway to Active. Your onboarding link is ready to share.

The system creates a unique onboarding URL for your practice. Share this link anywhere — it's your permanent patient registration page.

Patient completes everything

When a patient opens the link, they flow through each step in order: registration, consent, admin forms, then assessments. All responses land on your dashboard grouped under the patient's record.

Frequently asked questions

Can I have different pathways for different types of patients?
Yes. Create one pathway for standard psychology patients, another for telehealth-only, another for NDIS participants. Each has its own link and can include different consent templates, forms, and assessments.
What happens if I change a pathway after patients have already used it?
Changes only affect future patients. Existing submissions are preserved. If you change the consent template, patients who already signed the old version keep their signed copy.
