Edit a consent or practice form
Update the wording or sections of a form you have already set up. Consents already signed are not affected.

Save
Future clients
Open the form
Go to Client Forms and, in section 2 (Practice forms), click the form you want to edit.
Edit sections
Change the wording, add a section, or exclude one you no longer use. Each section shows whether it is Included or Excluded, so you can see what a client will be shown.
Save
Save your changes. A form that is already Active stays Active; a form still in draft becomes live when you press Save & mark reviewed.
Future clients get the update
Anything sent from now on uses the new version. Consents already signed are unchanged — a client who has signed keeps their original copy.
