The next
Open the registration form
Go to Client Forms and, in section 1, click your registration form. It is named Patient Registration unless you have renamed it.
Switch fields on or off
Each field has a switch, grouped under headings like About you and Contact. The preview on the right updates as you go. Name, date of birth and a phone or email stay locked on, because client matching needs them.
Add your own question
Add your first custom question to put something on the form that is specific to your practice. It joins your question library, so you can reuse it on any registration form you build later.
Save
Press Save. If registration is not switched on yet, the button reads Turn on registration instead and does both at once.
The next client sees the update
Anyone who opens your registration link from now on gets the new version. Answers already submitted are unchanged.
